Two health forms are required for each volunteer:

1. Health History Form -- to be completed by the volunteer

2. Medical Recommendation -- to be completed by a health care professional

Both of these forms should be completed and returned to the camp office at least FOUR WEEKS  prior to the volunteer's arrival at camp.
